TEMPORARY CLOSURE

OF THE  PATIENT LIST 

FOR NEW PATIENT REGISTRATIONS

AT DUNSTER AND PORLOCK SURGERY FROM 1 MARCH 2023


Our priority is to ensure that patients most in need of care can continue to access the support they need at our practice.

To help us provide our patients with the best possible care - from 1st March to 31st May 2023 - we are temporarily closing access to new patient registrations at our practice.

This means that during this period we will only be accepting new registrations from:

  1. immediate household family members of patients already registered at the surgery; or
  2. any patients permanently living in an area where there is no other GP practice available to register at.

New patients

We welcome new patients who live within our practice areas.  As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the pharmacist or doctor.   There is a health questionnaire 18+ and Children under 18 to complete along with a GMS1 form.

Medical treatment is available from the date of registration.

Patients registering at Dunster surgery may be eligible to use the on-site dispensary; see the 5 reasons for using the dispensary if you are invited to.

You can return your completed registration form and health questionnaire to somccg.reception-dunstersurgery@nhs.net or alternatively bring your forms into either Dunster or Porlock Reception desks, preferably with ID or proof of address.

Temporary Patients

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the practice and complete the form below.   You cannot register as a temporary patient at a practice in the town or area where you are already registered.

ONLINE PATIENT ACCESS

If you’re a patient at our practice you can now use the new NHS App, a simple and secure way to access a range of NHS services on your smartphone or tablet.

You can use the NHS App to get health advice, book appointments, order repeat prescriptions, view your GP health record and more.

If you already use online access, for example, Patient Access, you can continue to use it. You can use the NHS App as well.
